The different types of Vaginismus

Primary Vaginismus- vaginal insertion of any kind or first time attempts with penetrative sex have always been impossible or extremely painful. You may have noticed tampons were painful or impossible at the age of 12 when preparing to go swim. Or you likely discovered your vaginismus on the night of your wedding or your first attempts with sex.

Secondary Vaginismus- initial comfort and painless penetration with sudden loss of one's ability to achieve penetration. This may happen after: a traumatic birth or pregnancy, after experiencing sexual trauma, after recurrent painful vaginal infections, or after prolonged sexual inactivity or hormonal insufficiences. 

Situational Vaginismus- pain free vaginal insertion in one situation (like pelvic exams or sex) but inability to insert one's finger or inability to achieve tampon insertion.

What are some of the causes vaginismus?

  • Unknown causes- some people have vaginismus without any known reason.
  • chronic general anxiety
  • sexual anxiety or a fear of pain with sex
  • history of sexual trauma or physical trauma to the vagina
  • a fear of your own vagina
  • recurrent UTI's or vaginal infections that cause pain
  • endometrios, fibroids, or severe abdominal pain
  • religious upbringing with negative views of sex
  • conservative cultural upbringings that discourage sex education
  • lack of sex education and understanding your body
  • painful or traumatic vaginal birth
  • vaginal injury with pain

How does our 8 week virtual treatment for vaginismus and sexual pain actually work?

We are pelvic floor physical therapist who focus on vaginismus and pain with sex. We use our expertise, education and training to use the help of pelvic models, vulva models, dilators, pelvic wands, and clear instructional demos to teach you exactly how to do three major skills in every coaching session with us.

  3. Use your own hands strategically for progressive dilation, pelvic wanding, and pelvic floor screening so you can feel in charge and in control of your body!

Why sex feels like hitting a wall, your vagina is too small, or that like a ring of burning pain at the opening? The physical barriers to vaginismus.

If you are feeling safe, have enjoyed foreplay, and are using lubricant- and still feel like sex is blocked OR that penetration is completely impossible, then you are experiencing the physical and pelvic floor muscle barriers to pleasurable sex. These barriers generally require physical interventions to fix them!

This is due to significant pelvic floor tightness that can can make the vagina feel small, barely there, and of corse- extremely painful with insertion. This can be reversed! How do I know if my vaginismus is due to a mental block and not a physical barrier?

Due to the significant emotional distress, isolation, self insecurities, and shame that vaginismus may cause- getting mental health support is honestly an avenue that everyone with vaginismus should pursue. But if you are experiencing flashbacks of sexual trauma, feel like you are shutting your thighs before an attempt at penetration due to fear of pain, or if you go into panic before intercourse attempts, you should consider seeing a mental health therapist to help you through these feelings.
